Background: Competency based medical education (CBME) relies on supervisor narrative comments contained within entrustable professional activities (EPA) for programmatic assessment, but the quality of these supervisor comments is unassessed. There is validity evidence supporting the QuAL (Quality of Assessment for Learning) score for rating the usefulness of short narrative comments in direct observation.

Objective: We sought to establish validity evidence for the QuAL score to rate the quality of supervisor narrative comments contained within an EPA by surveying the key end-users of EPA narrative comments: residents, academic advisors, and competence committee members.

Methods: In 2020, the authors randomly selected 52 de-identified narrative comments from two emergency medicine EPA databases using purposeful sampling. Six collaborators (two residents, two academic advisors, and two competence committee members) were recruited from each of four EM Residency Programs (Saskatchewan, McMaster, Ottawa, and Calgary) to rate these comments with a utility score and the QuAL score.  Correlation between utility and QuAL score were calculated using Pearson’s correlation coefficient. Sources of variance and reliability were calculated using a generalizability study.

Results: All collaborators (n = 24) completed the full study.  The QuAL score had a high positive correlation with the utility score amongst the residents (r = 0.80) and academic advisors (r = 0.75) and a moderately high correlation amongst competence committee members (r = 0.68).  The generalizability study found that the major source of variance was the comment indicating the tool performs well across raters.

Conclusion: The QuAL score may serve as an outcome measure for program evaluation of supervisors, and as a resource for faculty development.

A continuing professional development imperative? Examining trends and characteristics of health professions education doctoral programs  | Vocational education and training - VET | Scoop.it

Despite the long-standing faculty development initiatives for improving teaching skills in the health professions, there is still a growing need for educators who are formally trained in educational theory and practice as health professions schools experience dramatic demand and growth. Graduate programs in health professions education (HPE) provide an avenue for health professions’ faculty continuing professional development to enhance their knowledge and skills for teaching and curriculum leadership roles. There has been a proliferation of certificate, master’s, and doctoral programs in HPE over the last two decades to respond to the growing need for well-prepared faculty educators and program leadership. The purpose of this study was to identify and describe current HPE doctoral programs in United States (U.S.) and Canada.

Conclusions
The workforce shortage facing health professional schools presents an opportunity, or perhaps imperative, for continuing professional development in HPE through certificate, master’s, or doctoral programs. With the current exponential growth of new doctoral programs, there is a need to standardize the title, degree requirements, and further develop core competencies that guide the knowledge and skills HPE graduates are expected to have upon graduation.

As the United States faces an increasingly aging population and its current nursing workforce ages into retirement, the demand for nurses is expected to be acutely felt in some regions of the country, particularly in rural communities where many patients already must travel long distances for care. According to the American Nurses Association, approximately 690,000 nurses are expected to stop working by 2024. Further compounding the problem, nursing programs are struggling to meet rising demands due to constraints on class size and the number of training spots available. At the same time, there is also a higher demand for a more culturally diverse and multilingual nursing workforce as demographics shift across the U.S.

Expansive Learning at Work: Toward an activity theoretical reconceptualization

Activity theory and its concept of expansive learning are examined with the help of four questions: 1. Who are the subjects of learning? 2. Why do they learn? 3. What do they learn? 4. How do they learn? Five central principles of activity theory are presented, namely activity system as unit of analysis, multi-voicedness of activity, historicity of activity, contradictions as driving force of change in activity, and expansive cycles as possible form of transformation in activity.

Meet Me in the Metaverse: How the continuum of technology and experience is reshaping healthcare

As a doctor checks on his patients in the hospital, he
notices one woman is healing very slowly from a simple
laparoscopic surgery. The clinician takes his concern to
the metaverse, going back in time to witness a previous
abdominal surgery this patient underwent. He pulls up
this surgical video to show the patient and they both
see that the area that is healing slowly had developed
scar tissue from this former surgery. A week later after
discharge, the patient seamlessly carries her personal
health data to a physical therapist who demonstrates
exercises in an immersive environment simulating a
tropical beach setting. At the end of the session, the
therapist fast forwards to the future to show the patient
how she will be moving six weeks from today. This is the
future of the healthcare metaverse.

Denmark. Modeling Enrollment in and Completion of Vocational Education: The Role of Cognitive and Non-Cognitive Skills by Program Type

This study provides evidence of the importance of cognitive and non-cognitive skills to enrollment in and completion of three types of vocational training (VET): education and health, technical, and business. Math and language exam scores constitute the key measures of cognitive skills; teacher-assigned grades the key measure of non-cognitive skills.Estimation of completion proceeds separately by gender and VET type, controlling for selection and right censoring. The authors find that all skills are inversely related to VET enrollment, results that are robust to family-specific effects. Estimates for completion vary considerably by program type, demonstrating the methodological importance of distinguishing among different VET courses. While math scores are positively related to certification for all VET tracks, language skills are more important for the nontechnical track, and non-cognitive skills appear important only for the business track.

They got mad skills : the effects of training on youth employability and resilience to the Ebola shock 

This paper discusses a randomized control trial to measure the short-term impacts of a skills intervention among urban youth in Sierra Leone at the onset of the Ebola crisis. The intervention provided (i) technical skills training, plus on-the-job training; (ii) business skills training; and (iii) a mix of (i) and (ii). All groups received stipends and literacy and numeracy training. The findings support evidence that combining cash injections and skills training can stimulate employment and entrepreneurship. The program boosted household consumption and investments in housing and assets, thereby building resilience to the Ebola shock.
